The muscles of the deep group of the posterior (crural) compartment of the leg lie deep to the soleus muscle. To expose the deep muscles, the soles must be cut and reflected. Cut the soleus muscle along its attachments to the tibia and fibula, and then reflect it and the gastrocnemius muscle distally towards their insertion via the calcaneal tendon into the tubercle of the calcaneus .
Open the intermuscular septum, which divides the posterior compartment into the superficial and deep muscle groups. Identify the tibial nerve [two probes; retractor holds the soleus muscle] and accompanying posterior tibial vessels [two probes].
