On the posterior aspect of the superior vena cava, identify the arch of the azygos vein [cross-section]. It passes superior to the root of the right lung and joins the superior vena cava. Cut the superior vena cava (svc) just superior to the entrance of the azygos vein and reflect it superiorly  . Links and References:
