Step 1. Incisions of the Abdominal Wall

Make a vertical incision, slightly to the left of the midline from the umbilicus to the xiphoid process (Figure 36.2). The lower abdominal wall has already been studied ( see Laboratory Thirty-Six). Reflect the lower triangular portion of anterior abdominal wall inferiorly .

Next, make an incision along the left costal margin to the midaxillary line and reflect the left flap of the anterior abdominal wall laterally . Before reflecting the right flap of the anterior abdominal wall , identify and detach the round ligament of the liver (ligamentum teres) from the internal surface of the umbilicus. Next, sever the attachment
of the falciform ligament of the liver from the internal surface of the anterior abdominal wall . Cut along the right costal margin of the midaxillary line and reflect the right flap of anterior abdominal wall laterally.
