Clean the sternocleidomastoid muscle and carefully reflect it superiorly to avoid damaging the associated nerves .

Identify the spinal accessory nerve (CN XI) as it enters the deep surface of the sternocleidomastoid muscle. Trace the spinal accessory nerve superiorly. It exits the base of the skull through the jugular foramen with the internal jugular vein, vagus nerve (CN X) and glossopharyngeal nerve (CN IX).
