Make a vertical incision, slightly to the left of the midline from the umbilicus to the xiphoid process (Figure 36.2). The lower abdominal wall has already been studied ( see Laboratory Thirty-Six). Reflect the lower triangular portion of anterior abdominal wall inferiorly .
Next, make an incision along the left costal margin to the midaxillary line and reflect the left flap of the anterior abdominal wall laterally . Before reflecting the right flap of the anterior abdominal wall , identify and detach the round ligament of the liver (ligamentum teres) from the internal surface of the umbilicus.
